To apply for public assistance benefits you can come directly to our agency. We have staff available Monday through Friday from 8 a.m. to 4:15 p.m. to take your application and interview you for benefits. We will accept your application until 4:30 p.m. Walk-ins are seen on a first come basis so we encourage you to come early in the day, if possible.
You can complete an application through MNBenefits online. After you complete the online application it will automatically be sent to our agency. We will be in contact with you by telephone or mail.
Additionally, you may download and complete the Combined Application Form (PDF). Completed forms can be mailed or faxed to the Financial Assistance Division at 218-327-5548. You can also drop off an application in person, email or in the drop box (after hours). Our address is:

We can complete a telephone interview with you for the majority of our programs, however if you come into the agency, we can interview you on the same day and determine if you qualify for expedited processing. You may reach us by phone at 218-327-2941.
If you are applying for health care benefits, you can apply directly online through the MNsure website. We can also mail you an application packet if you call or email your request.
