Providers
- Read your Provider and Pharmacy Directory (PDF) or go to 2024 MSHO Provider and Pharmacy Directory (PDF) to see a full list of health care providers (including specialists) in the IMCare Classic (HMO SNP) network.
- Use our online search feature to see if a specific pharmacy providers is in the IMCare Classic (HMO SNP) network.
- When you are a member of IMCare, you must use network providers except in emergency or urgent care situations, or for open access services, out-of-network dialysis services, or any other services previously authorized. If you obtain routine care from out-of-network providers neither Medicare nor IMCare Classic will be responsible for the costs.
- As a member of IMCare, you will be assigned a care coordinator. Your care coordinator will work closely with you, your doctor and any other support people you choose to make sure you receive the services you need. They also work with you to arrange other services you may need, such as dental care, preventive care, home delivered meals, and chore services.
