Diabetes is the condition in which the body does not properly process food for use as energy. Most of the food we eat is turned into glucose, or sugar, for our bodies to use for energy.  When you have diabetes, your body either doesn't make enough insulin or can't use its own insulin as well as it should. This causes sugars to build up in your blood. This is why many people refer to diabetes as “sugar.”

Diabetes can cause serious health complications including heart disease, blindness, kidney failure, and lower-extremity amputations. Diabetes is the seventh leading cause of death in the United States.

IMCare offers diabetes disease management to members 18 years and older through our Ways to Wellness program. This educational program can teach you how to manage your diabetes.  Ways to Wellness member education for diabetes includes the following topics:

  • Nutrition therapy
  • Physical activity
  • Weight management
  • Education for self-management
  • Foot care
  • HbA1C
  • Blood pressure
  • Aspirin use (as recommended by PCP)
  • Dilated eye exam
  • Annual history and physical exam
  • Renal assessment

Membership in the Ways to Wellness disease management program is voluntary.  This service is a benefit and there is no charge. If members choose to enroll, their covered services will remain the same.  When enrolled members get:

  • Support from a registered nurse
  • Education about your condition
  • Information on covered services        
  • Self-management techniques
  • Coordination of care

Education is provided over the phone or through the mail. Members don’t need to leave home to receive the services.
